Erie County NY Connects is a person-centered system of information and resources to make it easier for individuals of all ages to objectively learn about and access long-term services and supports. Often known as a “one stop shop” or Single Point of Entry Program, Erie County NY Connects is designed to:
· serve as visible and trusted sources of information on the full range of long-term care options that are available in the community, including home-based, community based and institutional care;
· provide personalized and consumer friendly assistance to empower people to make informed decisions about care options;
· attempt to provide coordinated and streamlined access to all long-term care options so that consumers can obtain the care they need through an assessment and eligibility determination process;
· help people to plan ahead for their future long-term care needs; and
· assist people in accessing benefits and programs for which they are eligible.
